How to apply
To qualify for consideration for a financial award or assistance from The Sharanpal Singh Charitable Trust for Kidney Patients, you need to fulfil the following conditions:
- Show that you are a sufferer of renal failure, have kidney disease or a related condition fitting the above criteria, or that you are an institution as that fit’s the above criteria.
- Complete the Application Form
- Provide a referee, who can complete the Referee Form
- Provide any other requested documents
- On a successful award, ensure that the Award Completion Forms are completed either by you or your referee and returned to us promptly once the funds have been delivered and used by you.
Referee:
- Individual awards. The referee should be the institution where the treatment will be administered
- Institution awards. The referee should be a trustee or similar of the institution.
